Heartland Alliance / HIV Housing Advocacy Program (HAP) provides a housing advocate assigned to each client which provides assistance with housing location and rental payment.

Prerequisities

Must have a HIV/AIDS generated disability.
Income should not exceed 50% of median family income for their area of residence.
Live in an apartment meeting fair market rent levels.
Capacity to live independently.
Monthly rent or mortgage should not exceed monthly income.
Clients have to be referred through AFC (Coordinated Housing Advocacy Program of the AIDS Foundation of Chicago) either by calling AFC at 312.922.2322 or through their coordinated case manager.
