Heartland Alliance / Supportive Services for Veteran Families provide case management for housing stability, location and inspection, emergency financial assistance to obtain or sustain permanent housing, renter rights and responsibilities education and support, asset development and financial education, employment readiness and placement, referrals for health care, wellness, transportation, legal services, childcare, daily living.

Prerequisities

Must reside in the City of Chicago
income less than 30% of AMI (Area Median income)
Meet one of the following criteria:
Be at risk of losing permanent housing within 2 weeks (i.e. have eviction notice or scheduled eviction court date)
Have notarized letter from lease holder that states need to vacate unit within 2 weeks
Reside in housing not suitable for human habitation (park, street, condemned housing)
Fleeing or willing to flee a domestic violence situation
REFERRAL AND INTAKE
Photo Identification
DD-214 Form showing Discharge status (SSVF can’t serve veterans with dishonorable discharge status)
Proof of any income of household
Housing payment receipts if currently housed
Copies of lease/landlord agreement if currently housed
Social security cards of household members
Birth certificates of dependent children
