Provides permanent housing, rental subsidies, case management
Chronic Homeless Initiative (CHI), Permanent Support Housing Program
Provides initial evaluation, referrals to services such as metadone clinics, outpatient services, self-help groups
Provides part-time employment and training, vocational counseling, housing assistance
Social Security Administration funded Representative Payee Program
Provides case management, financial counseling, budgeting, linkage to resources and housing
Emergency Assistance – emergency client assistance from FEMA, Emergency Food and Shelter Program, The Chicago Community Trust’s Jones Fund and The Emergency Fund
Eddie Beard Homeless Veterans’ Housing Program (Vet House)
Provides supportive services, case management, linkage to Homeless Veterans’ Reintegration Program (HVRP)
PH can house up to 13 individuals at a time.
Vet House can house up to 15 individuals at a time.
