Have fixed income or find employment within 90 days after intake
TB test done within the last 6 months
Valid Picture ID
Social Security Card for adults and Birth Certificates for children
Immunization records for all children
Must be compliant with medication if under mental health treatment
Requires referral from case manager
Provide 3 months of most recent paystubs, 6 months of most recent checking bank statements, 2 months of most recent savings bank statements
