Heartland Alliance / EPOCH (Every Positive Change) provides supportive housing program in scattered apartment units providing housing case management, referrals to substance abuse treatment, educational support groups, transportation services.

Must have income but has to be lower than 80% of median income of the area
A documented HIV/AIDS status
Current substance abuse or history of it
Be willing to pay 30% of income towards rent. Rent is based on HUD calculation
INTAKE requires:
tuberculosis testing
proof of HIV/AIDS status
Photo Identification
Integrated Assessment – its conducted by the program after intake
Proof of income Program does not accept walk-ins. Appointments are required to receive services
