Referral from recent treatment facility with at least 28 days of sobriety.
History of treatment.
Negative TB.
Medically stable and capable to attend all activities.
Intake :
Provide verification of HIV status signed by a physician.
Provide Negative TB results from the last 6 months. Must be signed by the medical provider.
Provide proof of homelessness and/or referral letter.
