Heartland Alliance / Family Assertive Community Treatment (FACT) provides housing, case management, linkage for women to employment opportunities, treatment, financial support and education, linkage for children to health care, education, therapy and developmental services .

Must be homeless or at risk of homelessness.
Mother must be single and of ages 18-25 with at least one child younger than 5 years old.
Mother must be diagnosed with mental health or substance abuse issues.
At least one child must show emotional, behavioral or attachment disorders,or developmental delays.
History of foster care or involvment with DCFS for the mother.
Must live in Chicago.
