past 30 days income, check stubs, current Fip Statement
current child support printout for 30 days/yearly
current Title XIX cards
Social Security statement/printout
Unemployment printout for past 30 days
veterans benefits
federal income tax return
rent receipt (with landlord’s name, address, telephone number)
current heat and electric vendor bills
Social Security cards for all household members

Homeless prevention
eviction notice (written documentation from landlord)
written rental agreement
foreclosure documentation
Social Security cards for all household members
due date if you are exiting a halfway house/treatment facility
documentation if your residence has been Red Tagged
proof of income for the past 90 days (job, benefits, etc)

Rapid Re-Housing
must reside in a shelter/transitional housing/place not meant for human habitation
must provide documentation from an outside agency on that specific situation
Social Security Cards for all household members
proof of income for the past 90 days (job, benefits, etc)

General Assistance
income at or below 100% of Federal Poverty guidelines

Angel Car
must live in Woodbury County
must be a single parent living with child(ren)
must have no operational vehicle
must have a job
must have a valid driver’s license
must be able to obtain vehicle insurance
must have a good driving record
must be able to pay for license plates, tax and registration fees
