Prerequisities

Emergency Assistance
Must live in Cook County
Must provide identification. Volunteers will help you acquire it if you do not have current identification
Bring Social Security cards, birth certificates, school and/or medical records, prison release documents, utility bill or lease, benefits award letter or other documents which qualify as proof of identification
Proof of income is required for some services
Documentation for financial issue, such as 5-day notice and lease copy, utility shut off notice and bills, appointment slips for medical visits or job interviews.

PADS Shelter documentation required:
Photo ID. You can stay only one night if you do not have it and must report to the Support Center the following day
Shelter does not serve unaccompanied youth or sexual offenders
